Please ensure the phone number you provide is not a restricted number. You will receive a call back by the covering provider within 30 minutes. If this time has passed and you have not received a call, please attempt to contact the on-call provider again.

Please contact your pharmacy for medication refills. If there are no refills available, your pharmacy will contact your provider for authorization.
